Dreamweaver MX 2004 Commands Menu Not Loading

If your Dreamweaver MX 2004 Commands Menu won't load, it maybe be b/c of a buggy extension.  In my case, an old version of AddFormmail - you can see the error starting on line 395.  The log was created using Filemon.
